Please include the following:

Billing and Shipping information, as it will be seen when the order is placed.

• Contact name along with fax and phone numbers.
• Item numbers with descriptions and quantities.
• Preferred pay method.
• With the fax number you provided, you will receive a quote on the total of the order including shipping and processing fees.
• Quotes are valid for 30 days from receipt.
